Patient Privacy
Confidentiality and Privacy:
Patient health information (PHI) is individually identified health information related to your past, present, and future physical and/or mental health.
The Home Health Agency is required by law to:

Keep your protected health information confidential;

Provide the notice of legal duties and privacy practices related to the use and disclosure of your PHI;

Provide you access to your medical record upon request, unless it has been determined by a health care professional that it would be harmful to you;

Follow the terms of the notice currently in effect;

Communicate any changes made to the policy;

Use/disclose PHI to provide, coordinate or manage your healthcare and related services. In emergencies, it will be used to provide the treatment you need;

Use PHI, as needed, to obtain payment for your health care services;
